Examples of Good/Bad Registration Forms

The following guide explains how to create eligible and correct registration forms on the Comet system. 


When uploading a registration form, the Comet regulations state that the player's full name, signature and date of signature (if the player is above 16) must be present on all registration forms. 


The date of the player's signature (or parent/guardian if the player is under 16) must be within 14 days of the submitted registration form on Comet. 



1. Example of a Bad Registration Form


In this example, the player's name is correct, however the signature is typed and not signed and the date of signature is outside of the 14 day period. 


In order to correct this, the signature must be signed and the date of signature will have to be within 14 days of the submission date of the registration form. 


If this is followed then the registration form can be approved. 





2. Example of a Good Registration Form


In the screenshot below is an example of an acceptable registration form. 


In this example, the signature of the player is clearly signed and the date of signature matches the date in the top left of the registration from (within 14 days of submitting). 


This registration form can be submitted and will be approved within 48 hours of submission if all other Comet regulations are followed. 


 



3. Example of Uncompleted Registration Form


This is another registration form that will not be accepted if submitted as all three sections need to be completed before submission. Once the player's name, signature and date of signature is added, the player can be approved.
